We will assess the needs and develop a comprehensive care plan. From there, we coordinate and manage various aspects of care, including medical appointments, therapy sessions, and home care services. We will ensure that all care providers are on the same page and that the individual’s needs are met effectively.
Our care managers conduct a thorough assessment to evaluate an individual’s physical, cognitive, emotional, and social needs. Based on these assessments, we develop personalized care plans that address the specific challenges and goals of the individual. We always collaborate with the client and their family to ensure the plan reflects their preferences and values. Remember, you are more than a client, you become family and we want that reflected in the created care plan.
Our care managers become the advocate for the client, ensuring their rights and wishes are respected. We communicate with healthcare professionals, legal representatives, families and other relevant parties to ensure the client’s voice is heard and their best interests are protected.
Support and education to family caregivers. We offer guidance on the challenges of caregiving, provide resources and referrals to support services, and help caregivers navigate thru the complex healthcare systems. We also offer emotional support and coping strategies to prevent caregiver burnout.

We can offer guidance on financial matters, including accessing benefits, managing insurance claims, and finding resources for funding long-term care. We provide referrals to elder law attorneys for assistance with legal matters such as advance directives, estate planning, and guardianship.
Housing and Transition Support
Our care managers help aging and disabled adults and their families navigate housing options, such as assisted living facilities, nursing homes, or home modifications. They assess the individual’s needs and preferences, provide information about available options, and coordinate the transition process.
